The Global Hawk that crashed in the UAE earlier this year (while returning from a mission over Afghanistan) suffered a control failure due to a bolt that had been installed backwards by Northrop Grumman. This caused a rod that controls the tail to fail, sending the plane into a crash. --Stephen V Cole
